Patient Forms

 

If you are attending your first appointment at New Genesis Center for Wellness Consultation, Colon Therapy, Massage therapy, Far Infrared Sauna, or any Aesthetic/Skin Care appointment, please print the appropriate forms, fill them out, and bring them with you.  

 

If you are unable to complete and bring the forms with you to your appointment, please arrive 30 minutes early (for Wellness Consultations or Colon Therapy) or 15 minutes early (for Massage Therapy, Skin Care/Aesthetics, or Sauna) to complete them in the office.  

 

If you do not arrive in enough time to complete the forms prior to your appointment time, we may have to reschedule your appointment for another time.
